This form is intended only for medical and mental health providers who wish to refer ADULT clients for evaluations related to ADHD and autism.
Referrals for all other evaluations, including those for children, need to be made by calling the scheduling lines at (518) 744-7302 or (518) 415-5070.

Note: We require new patient paperwork to be completed before we schedule you for an appointment. Once your packet is completed, you will hear directly from scheduling staff with a phone call or text.
